The Five Numbers Your Annual Physical Doesn't Check


Your annual physical checks the basics. Blood pressure, cholesterol, blood sugar, weight. For most healthy adults, those numbers come back normal, the physician nods, and you leave feeling reassured.


That reassurance may be incomplete.


There is a category of metabolic markers — measurable, actionable, and clinically significant — that rarely appears on a standard annual panel. Not because they're exotic or experimental, but because routine primary care wasn't designed around them. They require specific tests, specific context, and a physician who is looking at prevention rather than treatment.


These five numbers don't replace your standard panel. They complete it.

1. ApoB — Apolipoprotein B


Standard cholesterol panels measure LDL cholesterol by weight. ApoB measures something more precise: the actual number of LDL particles carrying that cholesterol through your bloodstream.


Why it matters: two people can have identical LDL cholesterol levels but very different cardiovascular risk, depending on whether their cholesterol is carried in a small number of large particles or a large number of small, dense ones. Small, dense LDL particles are far more likely to penetrate arterial walls and initiate plaque formation. ApoB counts the particles directly, making it a more accurate predictor of cardiovascular events than LDL alone.


The American College of Cardiology now recommends ApoB for cardiovascular risk assessment, particularly in patients with metabolic syndrome or borderline LDL. It rarely appears on a standard panel.

2. HbA1c — Glycosylated Hemoglobin


HbA1c measures your average blood sugar over the previous two to three months, rather than a single fasting glucose reading taken on the morning of your test.


Why it matters: fasting glucose is a snapshot. HbA1c is a short film. Someone can fast perfectly the night before a blood draw and produce a normal glucose reading while spending the rest of their week in a state of chronic mild hyperglycemia — exactly the pattern that drives insulin resistance, metabolic syndrome, and eventually type 2 diabetes over years.


HbA1c catches this pattern early. In the pre-diabetic range, lifestyle intervention is highly effective. By the time fasting glucose is consistently elevated, the metabolic trajectory has usually been building for a decade.

3. Fasting Insulin


This is perhaps the most telling omission from the standard panel. Fasting glucose is tested routinely. Fasting insulin almost never is — despite the fact that insulin resistance, the underlying driver of metabolic syndrome, is often invisible in glucose readings until it is well advanced.


Why it matters: when cells begin to resist insulin's signal, the pancreas compensates by producing more. Blood glucose remains normal — for years — while insulin levels quietly climb. This state of hyperinsulinemia accelerates fat storage, particularly visceral fat, and is independently associated with cardiovascular disease, certain cancers, and cognitive decline.


A fasting insulin test, evaluated alongside glucose and HbA1c, gives a far more accurate picture of where someone sits on the metabolic spectrum than glucose alone. It is inexpensive, straightforward, and almost never ordered on a routine physical.

4. hs-CRP — High-Sensitivity C-Reactive Protein


C-reactive protein is a marker of systemic inflammation — produced by the liver in response to inflammatory signals circulating in the body. The high-sensitivity version of this test detects levels that are clinically significant but below the threshold of a standard CRP test.


Why it matters: chronic low-grade inflammation is now understood to be a central mechanism in cardiovascular disease, metabolic syndrome, and a range of chronic conditions. Elevated hs-CRP is an independent predictor of cardiovascular events — meaning it carries predictive value even in patients whose traditional risk factors look normal.


It is also modifiable. Diet, exercise, sleep quality, and certain targeted interventions can meaningfully reduce hs-CRP levels, making it not just a risk marker but a trackable outcome.

5. A Comprehensive Hormone Panel


Standard blood work rarely includes a full hormonal assessment. TSH (thyroid) may be tested if specifically requested, but estrogen, progesterone, testosterone, DHEA, cortisol, and their interactions are typically left out entirely — despite the fact that hormonal decline begins gradually in the fourth decade of life for both men and women and has wide-reaching effects on energy, body composition, cardiovascular health, bone density, and cognition.


Why it matters: hormonal changes rarely announce themselves dramatically. They present as fatigue that seems explainable, weight gain that seems reasonable, a gradual shift in mood or drive that seems like life. A comprehensive hormone panel gives you a baseline — and baselines matter, because the trajectory over time is what carries clinical meaning.

What "Actionable" Actually Means


The purpose of measuring these markers is not to generate anxiety. It is to generate a starting point.


Every one of the five markers above responds to intervention — dietary changes, targeted supplementation, exercise protocols, hormonal support, or simply more precise monitoring. But you can only act on what you know, and you can only know what you measure.


The standard annual physical is a valuable tool. It catches what it was designed to catch. These five markers require a program that is specifically built around prevention — one where the physician is asking not "what is wrong?" but "what is coming, and what can we change before it arrives?"
