The Hidden Epidemic: Insulin Resistance Before Diabetes

More than two in five adults in the United States now have prediabetes — and roughly eight out of ten of them don't know it [1]. Not because the condition is rare or hard to detect, but because the test that actually catches it early isn't part of a standard checkup. By the time a routine fasting glucose test flags a problem, the underlying process — insulin resistance — has often been building quietly for years.
This is the gap between feeling fine and being fine. Insulin resistance produces no symptoms at first. It doesn't show up on a scale, in an annual physical, or in how someone feels day to day. It shows up first in the blood, in a number most panels never order.

What Insulin Resistance Actually Is
Insulin's job is to help cells absorb glucose from the bloodstream for energy. When cells stop responding to it efficiently, the pancreas compensates by producing more — and for a while, that compensation works. Blood glucose stays in a normal range, a standard fasting glucose test comes back clean, and nothing about a routine physical looks unusual.
That compensation has a limit. Over time, as cells become progressively less responsive, the pancreas has to work harder to keep up, and insulin output eventually can't offset the resistance anymore. That is the point at which fasting glucose actually starts to rise — which is also, not coincidentally, the point at which most people first find out anything is wrong. Global data suggests this isn't a marginal issue: pooled estimates across dozens of studies put insulin resistance at roughly one in four adults worldwide [2].

Why It's Invisible on a Standard Panel
A routine physical typically checks fasting glucose, and sometimes HbA1c — both genuinely useful, but both measuring the downstream result of the problem rather than the problem itself. Insulin resistance is, by definition, the stage before glucose regulation breaks down. As the American Diabetes Association puts it plainly: doctors don't usually test for insulin resistance as part of standard diabetes care [3].
The more direct signal is fasting insulin, measured alongside fasting glucose and combined into an index called HOMA-IR — a calculation that estimates how hard the pancreas is working to hold blood sugar steady. It's a simple blood draw, but it isn't a default line item on most basic metabolic panels, which is why the people it would flag earliest are usually the ones who never get the chance to see it.

Why Catching It Early Actually Matters
This isn't just a question of an earlier diagnosis for its own sake. Research on beta-cell function — the insulin-producing cells in the pancreas — shows that by the time someone is formally diagnosed with type 2 diabetes, that function is already reduced by roughly half [4]. And the longer that decline continues unaddressed, the harder it becomes to reverse: in studies of dietary and weight-loss intervention, patients in the first few years of diabetes were substantially more likely to return to normal glucose regulation than patients who had been living with it for a decade or more, after which the underlying cellular changes become considerably more resistant to reversal [4].
The practical implication is straightforward. Insulin resistance, caught while the pancreas is still compensating well and before glucose itself has drifted out of range, is generally the most responsive point in the entire disease timeline — to nutrition changes, physical activity, sleep, and stress management, all of which have real evidence behind them for improving insulin sensitivity [5]. Caught only once fasting glucose is already elevated, the same interventions still help, but from a harder starting position.
